During my two weeks of attachment Khoo Teck Phuat hospital. One of the patients in my cubicle whom I took care of was Mr Zaim Fazl. His current weight is 66.6kg with a height of 173cm and has a BMI of 22.25kg/m². Mr Zaim is a male, Indian Muslim, aged 43 years old, with a second marriage, and 6 children under the age of 2 to 20 years old. 3 children from his previous wife and the other 3 children from his current wife. He is living in a jumbo flat at Bukit Panjang and his highest level of qualification of education was only secondary/ tertiary. Currently not working as he had been admitted for the past 6 months hence he left the job hence his current wife is the breadwinner.  He is currently diagnosed with obesity, and had done gastric banding in January 2017 with multiple complications such as slippage, adhesions, strictures, leakage, perforations and follow up surgery. Obesity is a condition where a person has accumulated so much body fat that it might have a negative effect on their health.  Team, T.M (2016, January 05). Based on Mr zaim’s case study, the social factor that brings about his medical condition is his socio-economic status (social factors are things that affect the manner of living.)

Thus, the social factor that brings about his medical condition is his socio-economic problem. Before doing the gastric banding surgery in 2006, Mr zaim worked as a real estate agent. Having to sustain only an O level certificate could not bring him to a job with decent pay to support his family, hence his ex-wife reckons that the salary he received is unable to raise the family and left the kids and her husband behind. Before his wife left him and the children behind, they often quarrel when he was at home. Hence, he often stays out late to drink with his friends during his free time and eating out lot most of the time. Being a taxi driver is strenuous and draining, especially when they had different shifts every day and had long hours for his shift, it messes up his sleeping routine. Furthermore, because of his shift, he often settled his meals on instant meals as he felt was convenient for him, also he consumes a large amount of caffeine. He could 3 cups of coffee a day to keep himself awake. At the same time because of his job, it is even more difficult for him to fork out time to exercise and hence leading him to gain excessive weight. In December 2016, he went to the doctor because he felt a sharp pain in his legs and found out that it was Osteoarthritis, causing severe health problems for him thus doctor recommended him to do gastric banding. However, there were multiple complications after the surgery

Furthermore, many articles also proven that people with poor socio -economic problem, such as poverty leads to greater risk of suffering obesity. This can be seen in one of the articles. It is stated that Poor families have limited food budgets and choices, and must often stretch supplies toward the end of the month. At the same time, other articles also prove by Business, Finance and Law Books. (n.d.). Retrieved January 11, 2018.The relationship between obesity and poverty has become more obvious and complex: being poor in one of the poorest countries may be associated with poor nutrition, while being poor in a developed country could mean a higher risk of obesity.  On the other hand alo due to the workload that he faced it is also proven that the long work hours caused his lack of physical activity. It is proven by straits time. They also found that the biggest increase in the proportion of overweight people came, unsurprisingly, during the transition to work. Lai, L. (2017, June 20).

Furthermore, there were articles about taxi drivers having higher chances of having obesity. It was stated by Lim, S. and Chia, S. (2018).  “A high proportion of the taxi drivers were obese” and he continue by stating that “drinking three or more caffeinated drinks daily and driving more than 10 hours a day”

3.    Conclusion

After Mr Zaim case study, I have learned to be more appreciative to my parents especially my father. The breadwinner of the family. Moreover, I have also learned that not everything can be bought with money with simple things such as happiness. During shift in the hospital, I had managed to have a conversation with Mr Zaim. He regretted not taking good care of his health when he was still healthy and physically fit. He wishes that more people out there would take effort to take responsibility of their health even with facing stress. Thus, I have decided to do something for Mr Zaim, I created a page on facebook. By using the social platform to share awareness to people around them. I relayed the thoughts from Mr Zaim on the page, and at the time I share some of my opinion and information I found online such as preventing from suffering obesity, or preventing from becoming one especially those from poverty.
